WK Pierremont · Care Management · Care ManagementShreveport, LANursingFull-Time, Day, 0800-1630Posted 01/08/2026Req # 8189The role of the Care Manager is to coordinate and manage care throughout the patient's acute inpatient illness; ensure continuity of services and appropriate utilization and management of resources. Demonstrate leadership abilities by exhibiting a proactive approach to the care management process. The case management processes, and key elements involved include planning, facilitating, advocating, educating, monitoring, evaluating and measuring the various outcomes for each identified patient population. Demonstrate commitment to group goals and teamwork. Assumes responsibility and accountability for own duties and performance. Participate in the health system's quality improvement initiatives, which include facilitation and implementation of the disease management processes, performance improvement, quality and outcome management.

QUALIFICATIONS:

Valid Louisiana nursing license (RN or LPN)Prefer a minimum of 5 years clinical experience in an acute care setting, preferably with management experience. Utilization and/or Case Management experience desirable.Prior experience and or knowledge of severity of illness/intensity of service criteria system, utilization or medical necessity/certification review preferred.
